We are looking for an Experienced/Autonomous Nurse Practitioner (CRNP) with a focus in primary care and urgent care to be a part of our team as we strive to provide better coordinated care management for patients in conjunction with our primary care department.

As a Primary Care Nurse Practitioner at Altheda, you will:
- Deliver comprehensive care, managing acute and chronic conditions through thorough evaluations, treatment plans, and follow-ups.
- Focus on preventive care, educating patients to proactively manage their health.
- Conduct diagnostic testing and screenings to detect health issues early.
- Collaborate with a multidisciplinary team to create personalized, evidence-based care plans.
- Refer patients to specialists when necessary and ensure continuity of care.
- Uphold regulatory standards and Altheda’s commitment to clinical excellence.
- Engage patients and families as partners in achieving optimal health outcomes.
Qualifications:
- Current Pennsylvania State License and DEA certification.
- 5+ years of experience in primary care or family medicine (preferred).
- A passion for patient education, preventive care, and holistic health.
- Strong clinical judgment and a commitment to high-quality care.
